Aikido New Member Application - for Families

1. The maximum Family Introduction you will pay is $84, and for a Family Annual membership, $266. The forms also allow for lesser amounts based on the number of family members joining and any special considerations we offer.

2. In the online application form in the space provided list all other family members who want to train and we will create their profiles. We must have: First Name, Last Name, M or F, Date of Birth.

3. It is an insurance requirement we have written signatures from ALL members of a family group confirming their acceptance of our Declaration, the wording can be found HERE. Please print and complete sections 1 & 2 on this PDF Membership Application form, for each person training, and give them to your instructor. A parent or guardian must co-sign the declaration for anyone under 18.

4. A minimum of two people are required to create a Family Membership.

Claiming a Student or Concession Allowance?
If you claim a Student or Concession allowance as part of this application, please supply the details in the application form. You will be required to upload a photo of the claimed card in the space provided.

Family Rate
On both the Annual and Introduction application forms you can select how many family members will be joining, with a minimum number of two people. There is a maximum any family will pay, however, if the dollar total of individual family members is under that amount select only the individuals to be included in the family membership.

Family Membership - independent records
When you join as a Family, the person applying becomes the 'administrator' for the family but everyone will have their own independent membership record. New family members can be included or deleted by the family 'administrator'.

Introductory membership
For people new to Aikido and may only be used once, renewing will automatically adjust to the annual fee.

Australian citizen/resident
Australian residency status impacts on automatic inclusion in our insurance cover. Please be sure to answer this question so any manual inclusion to the policy can be addressed.
